Organization Registration Forms

Organizations must download, read and closely adhere to the instructions posted below. They are critical to the successful completion, submission and processing of these forms. Corrupted or improperly completed and submitted forms will be returned to the sender, which will lead to substantial processing delays.

This form is available in three formats, one or more of which should be downloaded and saved before completion. Right-click the desired link and select "save link as" or "save target as". Do not attempt to complete the forms in your browser program.

The type of form used is at the discretion of the submitting organization, but only one should be emailed to R&FA (see note below).

Form choice note: Adobe Reader (.pdf) forms cannot be saved with data. They may be printed if a permanent record is desired, but an electronic version of the data must be forwarded to R&FA using the button at the top of the form.
